Care Continuity is hiring a Bilingual Care Navigator, fluent in English and Spanish, to help patients get the follow-up care they need after leaving the hospital.
You'll contact recently discharged patients and help them schedule their recommended follow-up appointments. For patients who accept our services, you'll call their physicians' offices to book those appointments. Your work helps patients make a smooth, healthy transition from hospital to home. This role suits someone who enjoys working independently on daily assignments, pitches in to support the team, and cares about patients. What You'll Do
- Make and receive calls with patients and provider offices in English and Spanish
- Schedule follow-up appointments on patients' behalf, based on their preferences and availability
- Review patient charts (written in English) to understand follow-up recommendations
- Verify insurance coverage using online tools and resources
- Send and receive medical records by fax
- Write clear, detailed notes in English on patient scheduling preferences and call outcomes
- Manage your daily workload independently and support teammates when needed
What You'll Need
- Fluency in English and Spanish, both spoken and written
- High school diploma or equivalent
- At least 18 years old
- Strong customer service and communication skills
- Comfort working across multiple windows and computer applications at once
- Ability to troubleshoot basic computer issues on your own
- A quiet, distraction-free home workspace and reliable high-speed internet
- A self-motivated, dependable, and organized work style, with strong multitasking skills
Nice to Have
- At least one year of college completed
- Experience in healthcare, a medical office, a call center, or customer service
- Familiarity with medical terminology or health insurance
