Job Description:

We are looking for a proactive and experienced Registered Nurse to join our virtual healthcare team in a clinical support role focused on referrals, diagnostic results, and post-visit follow-up.

In this role, you’ll help ensure that patients receive appropriate follow-up after their Rocket Doctor visits by managing referrals, reviewing lab and diagnostic results, supporting prior authorizations, and carrying out defined clinical tasks under physician guidance or approved protocols.

This is a part-time, remote position ideal for an RN who is comfortable working independently, managing multiple clinical workstreams, and operating in a fast-paced virtual care environment. The role requires strong communication, excellent attention to detail, and the ability to reliably manage time-sensitive follow-up tasks.

Responsibilities:

Monitor clinical inboxes, emails, and faxes for lab results, diagnostic findings, referrals, and patient communications
Review lab and diagnostic results and identify abnormal, urgent, or critical findings
Escalate urgent or critical results to the appropriate physician according to established protocols
Track abnormal results and follow-up tasks until they have been appropriately addressed
Carry out defined physician-directed clinical tasks under approved protocols or standing orders and within scope of practice
Communicate physician-approved follow-up instructions to patients
Perform post-visit clinical follow-up, including symptom checks, repeat testing coordination, medication-related follow-up, and other assigned clinical tasks
Manage and process incoming referrals from physicians and PAs
Triage and prioritize referrals based on urgency, completeness, and medical necessity
Research and identify appropriate specialists or clinics based on clinical need, location, insurance eligibility, and availability
Prepare and submit prior authorizations and insurance-related paperwork
Support patients with referral scheduling and locating facilities or specialists that accept their insurance
Track referrals through completion and help ensure relevant consultation notes and recommendations are returned to the clinical team
Maintain internal referral tracking systems, databases, and specialist directories
Accurately document all actions, communications, and follow-up plans in the EMR
Respond to patient inquiries related to referrals, lab results, and follow-up care
Support chronic care management and other longitudinal care programs as needed
Reliably monitor and respond to Slack, email, and assigned work queues during scheduled working hours
Ensure all practices are HIPAA-compliant and protect patient confidentiality
Participate in virtual team and medical meetings

Required Qualifications:

Active Registered Nurse (RN) license in the state where services are provided
2+ years of experience in a clinical nursing, ambulatory care, primary care, care coordination, referral management, or similar role
HIPAA certification
Strong understanding of medical terminology, medical specialties, lab result interpretation, and medication management
Experience reviewing and triaging clinical results
Experience with referrals, prior authorizations, insurance workflows, or care coordination
Experience with EMRs and familiarity with laboratory or clinical portals such as Quest/Quanum
Ability to carry out physician-directed tasks with professionalism, accuracy, and appropriate clinical judgment
Proficient with Gmail, Excel or Google Sheets, Zoom or Google Meet, Slack, Adobe PDF, and web-based healthcare portals
Comfortable working independently in a remote environment
Reliable home internet and a private workspace suitable for handling protected health information
Ability to troubleshoot basic technology issues independently
Exceptional attention to detail, organization, time management, and documentation skills
Strong written and verbal communication skills with patients, providers, and team members
Ability to adapt to changing workflows and follow standardized processes
Must be fluent in English; a second language such as Spanish would be an asset

Preferred Qualifications:

Previous experience in telehealth, virtual care, primary care, or ambulatory care
Experience with chronic care management or other longitudinal care programs
Experience carrying out clinical tasks under physician protocols or standing orders
Experience with referral coordination and closed-loop referral workflows
Experience with medication prior authorizations
Experience working with Medicare, Medicaid, and commercial insurance plans
